Checkout "Change Delivery Address" form
Vedang Mishra
What is the shop or customer experiencing?
First & Last Name Inputs: Both the First Name and Last Name fields use duplicate id="last_name" and name="last_name" attributes. Labels resolve to the same field, causing form readers to receive two "last_name" values.
Focus Management: Focus remains on the "Save Address" button after a failed submission instead of moving to the first invalid field.
What is the expected behavior?
Unique id and name attributes for First Name and Last Name inputs.
Label for attributes to match actual, existing input ids.
Keyboard focus to automatically shift to the first invalid input field upon a failed submission.
What error messages are there?
"First Name required." (passed as raw text inside aria-errormessage instead of as an element reference).
Did you replicate?
Yes. The issue was replicated on /checkout2025 by inspecting the DOM elements and attempting a form submission with empty required fields.
What settings were checked?
Form input HTML attributes (id, name, autocomplete).
Label for attributes for Street Address, Line 2, City, and Zip Code.
Form validation behavior (aria-errormessage attribute values and DOM focus placement on submit failure).
Provide examples, sku, order #, customer names, and screenshots where possible.
